Case Managers: Non-profit social service agency seeks qualified Journey and Senior Level Case Managers/Community Services Coordinators to conduct screening and assessments, make referrals for placement in substance abuse treatment, and provide follow-up services, including coordinating community services, developing employment and housing resources, maintaining participant files and documentation of services, collecting and analyzing participant data, conducting visits with treatment providers, and engaging in ongoing communication with program participants, treatment staff, parole staff, and county ADP staff. Positions available throughout the Bay Area, San Jose, and Sacramento. Journey and Senior Level positions are open in all areas.
The requirements for the positions include, but are not limited to the following:
„« Must possess a High School diploma or a GED;
„« Must possess a Substance Abuse Counseling Certification from a State of California Accredited Institution;
„« Must have a minimum of one year experience working in a substance abuse program serving criminal justice populations;
„« Experience providing supervision/direction to case managers is preferred;
„« Experience addressing substance abuse and criminal justice populations (state parole and probation) is necessary;
„« Experience with substance abuse treatment models and an understanding of treatment modalities is expected;
„« Knowledge of community resources and services is preferred;
„« Must be able to receive and maintain a California Department of Corrections and Rehabilitation security clearance; and
„« Must possess a valid driver's license and have a clean driving record.
